When they finally return to the reality, it has been completely destroyed by the nightmares.

@que Time is progressing, if the experience with the elf on the first chapters are to be trusted. Noah disappeared on the roofs and reappeared on the sewers. Same with the cow.

Assuming Noah didn't timeskip between laying down on the bed and waking up inside the dream, this sequence doesn't feel like it could have taken more than half an hour of real time, tops. The outside world is probably not much more screwed than when Noah left it.

Assuming nobody has tried to do anything intensely stupid.
